Personal Details

Let's begin with who you are and how to reach you. This information ensures your application is properly reviewed and your retreat experience is personalized from the start.

What would you like Coach Kay to call you during the retreat?

What do you do professionally? Feel free to share your title or describe your work.

Optional, but helps Coach Kay connect with you and understand your world.

During the retreat, Coach Kay and the team may take photos or short video clips for brand and community use. Please select your preference.

Section 02 of 08

Life Season & Identity

Before Coach Kay can serve you, she needs to understand the chapter of life you are currently in. This section gives her a window into your world as it is right now.

"The woman who knows herself cannot be shaken by anyone who doesn't."
— These questions are not about judgment. They are about understanding.

Not the highlight reel. The honest version. What is actually going on in your life as you complete this application?

Who are you? Mother, caregiver, creative, leader, survivor, seeker? How would you describe the woman you are at your core?

This could be a career move, a relationship ending, a loss, a healing journey, a reinvention, or anything that has significantly changed your life.

This question matters. It tells Coach Kay what strengths to build on during the retreat.

This helps Coach Kay understand your life context and available bandwidth for this kind of deep work.

You do not need to have it together to be here. This retreat is designed for women in the middle of the work, not after it is done.

Section 03 of 08

Relationship History & Patterns

This section helps Coach Kay understand the relationship landscape that has shaped you. You do not need to relive your past here. You only need to identify what has been true and what you are ready to change.

Your answers here are confidential. They are used only by Coach Kay to prepare for your retreat experience. You will never be asked to share anything publicly that you have not chosen to share yourself.

Not a list of names or timelines. A description of the emotional experience. What has love generally looked like for you?

If you could name one recurring dynamic that has shown up across more than one relationship or situationship, what would it be?

Select all that apply. This helps Coach Kay prepare trauma-informed facilitation for the group.

Not physical appearance. Emotional energy. What feeling or dynamic in a person tends to pull you in?

If you have thought about it, what early experiences, relationships, or beliefs do you think shaped how you show up in love?

Not what you have settled for. Not what you think is realistic. What do you genuinely want love to look and feel like in your life?

Section 04 of 08

Inner Healing, Confidence & Self-Worth

B.E.C.O.M.I.N.G. H.E.R. is an identity-shift experience. These questions help Coach Kay understand where you are in your relationship with yourself, so she can meet you exactly there.

1 = I am very disconnected from myself right now. 10 = I have a strong, loving relationship with who I am.

Very disconnected Strong and loving

Not the surface answer. Do you genuinely believe you are deserving of the love, care, and life you say you want?

The voice that shows up when you are about to take a risk, set a boundary, or step into something bigger. What does it say?

In relationships, with family, at work, with yourself. Are your boundaries clear? Inconsistent? Hard to maintain? Nonexistent?

If yes, how does your current support system complement what you are seeking from this retreat?

Knowing what has not worked is just as important as knowing what has. This prevents Coach Kay from repeating approaches that have not served you.

What did they say about how you show up, what your strengths are, or what you need to work on? Even a paraphrase is helpful.

This tells Coach Kay your existing strengths and what ground has already been covered.

Section 05 of 08

Retreat Goals & Desired Outcomes

What do you want to walk away with? These questions help Coach Kay ensure the retreat is built around real outcomes, not just an experience. Your answers directly shape how the week is facilitated.

If you could leave this ship having achieved one thing, what would it be?

Write this in the present tense as if it has already happened. What do you see? How do you feel? What is different?

The thing you know is there. The thing you have circled around but not fully faced. If it came to mind just now, that is probably it.

Deep transformation work is most effective when the basics of life are relatively stable. Coach Kay wants to make sure you are in a position to actually receive this work.

1 = I am still deciding how deep I want to go. 10 = I am fully committed to the full experience, no matter what comes up.

Still deciding Fully committed

Group Dynamics & Coaching Readiness

This retreat is a curated, intimate group experience. These questions help Coach Kay ensure the group is well-matched and that she can support everyone well. There are no right or wrong answers here, only honest ones.

Are you someone who tends to lead conversations? Stay quiet until you feel safe? Observe before opening up? Share openly and quickly?

Coach Kay needs to know how to approach you. Do you want gentleness, directness, or somewhere in between?

Every woman in the room is both a student and a teacher. What energy, perspective, or experience do you bring?

Name it. Coach Kay would rather know your hesitation before the retreat than during it.

Section 07 of 08

Health, Accessibility & Travel Logistics

These questions exist purely to serve you well. Your comfort, safety, and accessibility matter. If anything here requires a more personal conversation, Coach Kay and the team are available to speak with you directly.

This cruise departs from a US port and visits international destinations. A valid passport is required.

This helps the team tailor the onboarding and orientation experience for first-time cruisers.

Foundation tier guests may share cabin arrangements. If you are traveling with someone or have a specific preference, note it here.

Health & Accessibility

This information is used strictly to plan for your wellbeing. It is never shared publicly.

We will do our best to coordinate with the ship's culinary team on your behalf.

Do you have any mobility, sensory, or physical needs we should be aware of when planning sessions and venues?

This is completely optional and confidential. If you are currently in an acute mental health situation, we want to ensure this retreat experience is appropriate and supportive for you.

Section 08 of 08

Final Reflection & Commitment

You are almost finished. These final questions are the ones Coach Kay reads most carefully. They give her a complete picture of who you are and what you are stepping toward. Take your time here.

This is not just a retreat. It is a declaration. The act of completing this application is itself a statement about who you are becoming. Coach Kay reads every word.

Not what you think sounds right. What do you actually need? Accountability? Gentleness? Permission to be honest? Someone to finally call you higher? Tell her directly.

Not the polished version. The real answer. What happened, or what reached a point of enough, that brought you to this form?

Anything you want to make sure she sees, understands, or holds space for. Consider this your personal note directly to her.

Do not overthink it. What comes up first is usually the truest.

Name the resistance. The part of you that says you are not ready, not worth the investment, or not sure. What is it saying?
